REGISTER MY ATHLETE

All Mustang Football players must follow AIA and Peoria Unified School District policies regarding athletic participation and complete a physical and required documents.   AIA FORMS LINK

For 2026-27 School year, AIA Physicals must be done by one of the following: MD/DO/NP/PA ONLY.  Physicals taken on or after March 1, 2026 are good for the 2026-27 school year.

Once the physical has been completed, visit RegisterMyAthlete.com to register your player for the 2026-27 season. Upload or complete the appropriate documentation on Register My Athlete. Once completed and submitted, it will be reviewed by the SMHS Athletic Department. Please contact the SMHS Athletic Department for any questions related to Register My Athlete.

EXTRA CURRICULAR PARTICIPATION FEE

All PUSD athletes are subject to the Extra Curricular Participation Fee of $100 payable at the SMHS Book Store or online through InTouch. 

2026-2027 PLAYER PLEDGE FEE

The Player Pledge is $450 and covers the cost of helmet conditioning & decals, uniforms, game socks, compression shirts, walk through apparel, season events, banquet awards, practice equipment reconditioning and purchase, senior retreat, senior player banners, and much more!  Option to pay are below.
